It is one of the most important facts in orthodontics and one of the most commonly underestimated. Once braces come off or aligner treatment ends, the teeth have reached their planned positions but the bone and tissue surrounding them have not yet fully stabilised.
Without a retainer holding them in place, teeth will begin to shift back toward where they came from. In some cases this happens within weeks. In most cases it is a gradual process that becomes noticeable over months or years.
At KissDent in Tirana, retainers are fitted on the same day treatment ends. We do not send patients away to organise retention elsewhere or leave it as an afterthought. It is planned from the start of treatment and executed on the last day of it.
Why Retention Is Not Optional
Every orthodontist will tell you the same thing. The retention phase is as important as the active treatment phase. The difference is that during active treatment the results are visible week by week, which keeps patients engaged. Retention is invisible progress, which makes it easy to deprioritise.
The biology is straightforward. Teeth are held in the jaw by the periodontal ligament, a network of fibres that stretches and compresses as teeth move. When orthodontic treatment ends, those fibres have a memory. They pull toward the original tooth position. A retainer counteracts that pull consistently enough and for long enough that the fibres eventually reorganise around the new position.
Skip the retainer and the fibres win. It is not a question of whether teeth will move without retention. It is a question of how much and how quickly.

How Long Do You Need to Wear a Retainer?
The honest answer is indefinitely, or at least for as long as you want your teeth to stay in their current positions. The first six to twelve months after treatment are the most critical, when the risk of relapse is highest. After that, the risk reduces but never disappears entirely.
We recommend nightly wear of removable retainers on an ongoing basis for all patients. This is a small and unobtrusive habit that protects a significant investment of time and money.
Patients who stop wearing their retainer after a year or two and then notice their teeth have shifted are not experiencing something unusual. They are experiencing exactly what the biology predicts.

Types of Retainers We Offer
Fixed retainers
Fixed retainers are thin wires bonded to the back surface of the front teeth, completely invisible from the outside. They work around the clock without any action required from the patient and are particularly effective at preventing the front teeth from rotating or spacing out after treatment. Fixed retainers are our default recommendation for the upper and lower front teeth in most cases.
Removable retainers
Removable retainers are clear plastic trays similar in appearance to aligner trays. They are worn at night and removed during the day. They cover more of the dental arch than a fixed wire and are particularly useful for maintaining overall arch shape and bite position after treatment. For most patients we recommend wearing removable retainers nightly on an ongoing basis, not just for the first few months.
Combined retention
Combined retention using both a fixed wire and a removable tray is what we recommend for the majority of patients. The fixed wire handles the day to day stability of the front teeth. The removable tray maintains the broader arch and bite relationship. Together they provide more comprehensive protection than either approach alone.

The retention requirements are the same regardless of which orthodontic system was used. Whether your teeth were moved by metal braces, ceramic braces, Nuvola aligners, or any other clear aligner system, the underlying biology of tooth movement and relapse is identical. Retention is always needed.
Some patients assume that because aligner trays are similar in appearance to retainer trays, the last few aligners can serve as retainers. They cannot. Aligner trays are designed to move teeth, not hold them.
A dedicated retainer tray is fabricated from a different material to a different specification for a different purpose.

I went to Kissdent to see about old crowns (30 years old) and recent work that I had done in the EU that I was not happy with. With a panoramic xray (and visual aesthetic check) it was clear what needed to be done.
Fast-forward 2 weeks and I write now, with 9 beautiful new zirconium crowns. They are mostly back molars but one that I worried about most was my left front tooth. I have a difficult tooth colour to match.
Kissdent staff have done a wonderful job. The clinic and lab are located on the same premises so they are able to adjust the tooth colour and overall bite precisely.
All excellent quality work and material for a very reasonable price. Unbeatable in Europe and North America.

If you have noticed shifting after stopping retainer wear, contact us to assess whether a new retainer can recover the position or whether further treatment is needed.
Contact us as soon as possible to arrange a repair. A loose fixed retainer is not holding your teeth in place and teeth can begin to move quickly, particularly in the first years after treatment. Do not leave it unaddressed.
Use interdental brushes or superfloss to clean around and beneath the wire daily. We provide full cleaning instructions at your fitting appointment and check the retainer condition at follow-up visits.
